Consent & Privacy Notice

This triage application is a capstone proof-of-concept project developed to demonstrate how patient intake information can be securely collected and reviewed by authorized healthcare providers. The design of this system follows privacy and security principles outlined in Alberta’s Health Information Act (HIA).

1) What we collect

  • Identifying information (e.g., name, date of birth, contact details)
  • Health information you enter (e.g., symptoms, injuries, relevant medical history)
  • Optional identifiers used by health services (e.g., Alberta Personal Health Number / PHN, if requested)

2) Why we collect it

We collect this information to support triage and intake workflow and to help authorized healthcare providers understand your reported concerns before or during assessment. A healthcare professional may review, correct, add to, or override submitted information as part of care.

3) Legal Authority

This application is designed to align with Alberta’s Health Information Act (HIA). Under the HIA, healthcare organizations (“custodians”) may collect health information that is necessary for providing health services. Only information directly related to your care and triage assessment is collected.

4) Who may access it

Access is intended to be limited to authorized healthcare providers involved in your care (for example, triage nurses, physicians, and designated staff), and only on a need-to-know basis.

5) Safeguards

This application is a proof-of-concept and is not deployed within a live healthcare environment.

In a production healthcare setting, Alberta’s Health Information Act (HIA) requires that reasonable administrative, technical, and physical safeguards be implemented to protect health information. These safeguards would typically include encrypted transmission, access controls, authentication mechanisms, and audit logging.

6) Use and disclosure

Information is intended to be used and shared only for care-related purposes, with your consent where required, or as permitted or required by law (including urgent situations where safety is at risk). Your information is not used for marketing purposes.

7) Retention

Health records are retained in accordance with applicable professional and regulatory standards in Alberta. Medical record retention standards generally require records to be maintained for at least 10 years from the date of last entry for adults, and for minors, 10 years after the last entry or 2 years after the patient reaches age 18, whichever is longer.

8) Your rights

  • You can request access to, and correction of, your health information in accordance with the HIA.
  • You may ask questions about how your information is used for your care.
  • You may raise concerns with a privacy office or designated privacy contact.

9) Privacy Contact

This application is a capstone academic project and is not affiliated with a healthcare organization. No real patient records are stored or used for clinical decision-making.

In a real healthcare implementation, a designated Privacy Officer would be responsible for responding to access requests, corrections, and privacy concerns in accordance with Alberta’s Health Information Act (HIA).
